Patient Forms
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon 'Get Acrobat® Reader'. This will take you to Adobe's website and enable you to download the Acrobat program to your computer.

Insurance & Financing
Our office accepts Cash, Checks, MC, VISA, Discover and AMEX as forms of payment. For surgery that is not covered by insurance we also offer a flexible payment plan. Copays, coinsurance, deductibles and refraction fees are due at the time of service unless previous arrangements have been made.
We are a participating provider with Medicare, and as a convenience to our patients, we are also on the panel of many insurance companies, including HMO's or PPO's. If you are on any of these plans, Dr. Ashkanazy will accept the fees paid by the insurance company after payment of the co-payments and deductibles. If a referral is required for a specialist visit, it will be the patient's obligation to obtain this before the appointment. The following list is not complete, as it is always changing.
